This article explains how the original underground areas built under Al-Shifa in 1983, relate to the current situation:

A large portion of the buildings that now make up the hospital were built by Israel when it ruled Gaza. It pulled out of the strip in 2005, opening a window for Hamas to take control, and by 2007 the militants had begun building the command center under Al Shifa, the Israeli officials said.
At first, Hamas simply dug out areas off the original basements of Al Shifa’s buildings, later going deeper and adding floors and connecting it to the vast network of reinforced tunnels it was building across Gaza, the officials said. They said it has since grown into one of the hubs of a vast tunnel system that crisscrosses Gaza.

A former senior official at Shin Bet, Israeli’s internal security service, said both Hamas and Israeli intelligence referred to the network as “the Metro” and compared the compound under Al Shifa to a major station of the New York subway system.
The former Shin Bet official and two other Israeli officials said the compound included several floors with designated spaces for meetings, living quarters and storage facilities. It can hold at least several hundred people, they said.
So it's as I thought, the original areas have been expanded by Hamas, that makes sense now.

We are going to have to toughen up.

Shouting jihad, asking people if they are Jewish, straight to jail, and if not British then deport. The marches need to be reduced in frequency, and held in a showground, not central London which is for everyone to use.

Politicians need to get some backbone, and work out that accepting things is not helping, it’s pandering, and we will see people injured or killed.

U.K. is now looking for 6 terrorists, arrived in a small boat. Imagine explaining this to people from 1950.

Not naysaying anything you’ve written but I have also lost any hope regarding the toughen up part. Both the population and politicians actually.

In my mind there was never a doubt Hamas was using hospitals as operations centers, weapons depots and launching areas.

They would make a case that is morally permissible due to Israel's overwhelming superiority. I don't subscribe to that, but even if one does, once you cross that rubicon then everything Israel is doing (and then some) falls under the same broad moral justification.

The imbeciles bleating about the Palestinians being unfairly targeted by the Israelis would sing a different tune if their wife, mother, child were brutally raped, burned alive, shot execution style, kidnapped, etc. These fools spout their nonsense whether they be immigrants, students, politicians or professional hell-raisers from the safety of their cities and universities protected by the very institutions they rail against. Hamas would not suffer their stupidity for one moment.

The Israelis need to see this through, it will save many lives down the road. That does not mean I feel the death of an innocent Palestinian is any less tragic than an Israeli, a Brit, or an American and I recognize that some in the current Israeli leadership needs to face a day of reckoning. I only hope that eventually, some realization dawns on most Gazans that it was Hamas, and Hamas alone, that brought this hell to them.

My guess they realized that without medical treatment the hostages would die. And then, since they didn't want to just leave them in hospital for treatment, and probably to be rescued by IDF, they just decided to execute them - right there, in the hospital...

According to the local experts, nearly every building in Gaza has access to the tunnel system. After IDF will have finished with the tunnels, Gaza is going to look like Port-au-Prince in 2010...

A little glimpse of Israeli reality: an Israeli soldier wounded in Gaza is playing classic Arab music with an Arab doctor who treated him…

Hen pointed out that there are at least five hospitals closer to the border that Hamas could have taken the hostages to if they wanted them to receive medical treatment...

^^ That's why Sinn Fein have been trying to expel the Israeli ambassador to Ireland and refer Israel to the ICC (unsuccessfully, thankfully). PLO and Hamas famously aided the the PIRA with weapons and training during the Troubles. The PIRA at least weren't trying to wipe a country off the map, unlike their "friends".
