Doctors say he is suffered hypoglycemia (low blood sugar). Most common reason is overdose of insulin. Alcohol intoxication also can cause hypoglycemia in some cases.
Who would benefit most from narrative that it's poison?
I would say Navalny and his party are much better position today than yesterday. And for the other side the best option would be that it' "low blood sugar".
Personally I don't trust either at face value. In best case he is flown to neutral ground and we get to know if it was poisoning or not. But even if it was poisoning, we still wouldn't know who did it (I'm sure he has many enemies). So I think west shouldn't respond, with anything else than "let's have a thorough investigation etc.".
What would Putin stand to gain from poisoning an opposition leader that has posed barely any threat to him over the years (at most an embarrassment)? Nothing really. He could have him killed at any time, so why this specific moment when the focus is already on the region and Russia's role in it? Makes no sense from the Kremlin's perspective.
Someone he owes money or favors to is more likely imo if this isn't a political ploy by either his own party or a false flag by some foreign power.

I mean it could be poison too but ... bit early to jump to conclusions.
A now deceased neighbor had these type of seizures. His screaming could be heard early every morning. Before one of his relatives told us what had been really happening (after the guy's death), my housemate and I speculated that there must be someone trapped next door in some kind of Buffalo Bill basement pit, regularly calling for help.

Or the other way around. Somebody he pissed off. Even with Putin an unopposed authoritarian leader, it's about time to look for successor, and there are various power struggles within FSB to say nothing of the whole "Kremlin" world. I know he is a minor figure, but about two years ago Pyotr Verzilov a PR person for Pussy Riot was poisoned in a similar fashion.

The german physicians who examined him say he was very likely poisoned and is being treated for poisoning. Interestingly there has been a kind of media silence the last few days.
Now don't get this the wrong way, I have absolutely nothing against Nawalny, I'm rather annoyed at the German government (as usual) for its grandstanding during the initial days of this entire incident. Nawalny is basically a Russian ethno-nationalist with an anti-corruption streak to his political leanings. If his name were Albrecht or Karl-Heinz he would probably be a member of the AFD in Germany and Merkel wouldn't piss on him even if his whole body were to be on fire.
