HMS Ark Royal listing to starboard after being struck by a torpedo from U-81
Ark Royal operated in some of the most active naval theatres of the Second World War. She was involved in the first aerial U-boat kills of the war, operations off Norway, the search for the German battleshipBismarck, and the Malta Convoys. Ark Royal survived several near misses and gained a reputation as a 'lucky ship'. She was torpedoed on 13 November 1941 by the German submarine U-81 and sank the following day. One of her 1,488 crew members was killed. Her sinking was the subject of several inquiries, with investigators keen to know how the carrier was lost in spite of efforts to save the ship and tow her to the naval base at Gibraltar. They found that several design flaws contributed to the loss, which were rectified in new British carriers.

USN:

On 19 April 1989, an explosion occurred within the Number Two 16-inch gun turret of the United States Navy battleship USS Iowa (BB-61). The explosion in the center gun room killed 47 of the turret's crewmen and severely damaged the gun turret itself. Two major investigations were undertaken into the cause of the explosion, one by the U.S. Navy and then one by the Government Accountability Office (GAO) and Sandia National Laboratories. The investigations produced conflicting conclusions.

France:
Ironclad battleship Hoche, 1895. It suffered serious stability problems given her large superstructure and low freeboard as seen here. Yet it incorporated new technologies for the French Navy, like gun turrets for some of her main battery guns and compound armour plate.
USN:
Oliver Hazard Perry class frigate USS Simpson (FFG-56) pre 2004.
During Operation Praying Mantis in April of 1988, the Oliver Hazard Perry class frigate USS Simpson engaged the Iranian fast attack craft Joshon (Tiger/La Combttante class) after the Joshon fired a Harpoon missile at US ships. USS Simpson and USS Wainwright (CG-28) fired SM-1 missiles (in surface to surface mode) while the USS Bagley (FF-1069) fired a Harpoon. The Joshon was heavily damaged and was then sunk by naval gunfire.
