Colonel Jacob Waley Cohen, CMG, CBE, DSO, a member of the Royal Engineer Signal Service who served 41 Wing Royal Flying Corps and The Independent Force, Royal Air Force. October 1917 to November 1918 He also fought in the 2nd Boer War
He worked at the Royal Observatory, Greenwich, lived at Shalford Cottage Garden Close, Watford, Hertfordshire and of 5, London Wall, London. he died on the 12th July 1948 near St Bartholomews Hospital, London

This memorial is at the National Memorial Arboretum, Alrewas, Staffordshire
