1968 - US Army CH-47 Chinook (SN 66-19032) after a hydraulic failure at low level and high speed. No-one was killed, but everyone aboard was injured.

From Andy Dulay(pilot): My CH-47 crash in Vietnam (66-19032). This photo also showed up in an issue of Look magazine, circa 1968. The cause of the crash was a dual hydraulic failure at low level and high speed. We were inverted and traveling backward at about 50 knots by the time we hit the ground. Amazingly, nobody was killed, but everyone in the crew was injured. Photographer unknown.
