Born 1886 Sydney Australia to Charles and Catherine Blake of 9 Vista Street, Sans Souci, Sydney.
Drowned in the Bristol Chanel on the sinking of H.M Hospital Ship Glenart Castle 26 February 1918 aged 32.
Her father was granted a gratuity 12 February 1920.
Her service medals issued 22 August 1923
Commemorated on the Holybrook Memorial, Southampton, Hampshire.
