Born 1879 Newport, Monmouthshire to Mr and Mrs Daniel Guy, Elmswood, Newport.

Head Nurse at Auckland Willington and Shildon Joint Hospital, Tindale Crescent. Bishop Auckland, England. Former Superintendent Devonshire Hospital, Buxton.

Died of dysentry at Scottish Hospital (attached Serbian Army), Auxiliare, Salonika, Greece on 21 August 1916.

At rest in Salonika (Lembert Road) Military Cemetery, Greece.
